Follow the guidelines in the Google Form to pay national AMA dues and our local chapter dues. Take screenshots of your payments.
National Dues: $29 per year
*Use Zelle or pay in cash to complete local dues
Local Dues: $20 per year
Complete Application

Complete and submit the Google Form.
